Background:Many men are unable to achieve the desired erection of their sexual partner due to physiological or psychological causes. This may result in physical, psychological or both conditions. The use of tadalafil (Cialis) in men who are unable to achieve an erection of their sexual partner due to physiological or psychological causes has been investigated and it is found that men who have erectile dysfunction and/or sexual impotence are at an increased risk of developing erectile dysfunction in combination with sexual arousal. The aim of the present study was to investigate the impact of tadalafil on erectile function and its relationship with sexual impotence in men who have erectile dysfunction. The study was conducted among 3,965 men with erectile dysfunction who had completed the 12 months of the study. Tadalafil at a daily dose of 2.5 mg and for a maximum of 3 hours was administered to all participants. Erectile function (EF) and a number of physical parameters (i.e., blood pressure, blood sugar, cholesterol and triglyceride levels) were evaluated using the International Index of Erectile Function (IIEF) questionnaire before treatment and after 1 month and 3 months, respectively. The prevalence of erectile dysfunction was similar in both treatment groups (30.1% and 31.6% of the study participants, respectively), with higher prevalence in the treatment group (30.7%) compared with the control group (26.9%) (p = 0.001). The prevalence of sexual arousal (EF and IIEF) was higher in the treatment group (46.5% and 47.5%, respectively) than in the control group (45.5% and 44.3%, respectively) (p = 0.039). The prevalence of erectile dysfunction was significantly lower in the treatment group compared with the control group (33.9% and 34.1%, respectively) (p = 0.006). The IIEF questionnaire showed a similar pattern in the treatment group and control group, with higher prevalence in the treatment group (45.5% and 44.3%, respectively) (p = 0.001). The prevalence of sexual arousal was also significantly lower in the treatment group (26.9% and 25.5%, respectively) compared with the control group (26.9% and 22.4%, respectively) (p = 0.003). The prevalence of erectile dysfunction was significantly lower in the treatment group (30.7% and 30.6%, respectively) compared with the control group (29.4% and 22.4%, respectively) (p = 0.012). The prevalence of sexual arousal was also significantly lower in the treatment group (29.4% and 30.6%, respectively) compared with the control group (29.4% and 27.6%, respectively) (p = 0.003).
The prevalence of erectile dysfunction in the group treated with tadalafil was not different from the control group. A significant relationship between the prevalence of erectile dysfunction and sexual arousal was observed. Study design:A total of 3,965 men with erectile dysfunction were randomly selected in the present study from the online medical online clinics. The study was conducted in the urban area of Madrid (Spain). The study was conducted at a rate of 3.9 a day, between 3 pm and 9 pm on the same day. All participants were between 18 years of age and above who had not undergone any medical or dental treatment. The study was approved by the Ethics Committee of the General Hospital of Madrid (number 1619-15). Informed consent was obtained from the participants before the start of the study.

• Anti-viral medication:- telaprevir, atazanavir, retonavir, indinavir, saquinavir and cobicistat
• Recreational drugs:- amyl nitrate (poppers)
